HNS Policy 


All providers must obtain a current copy of each patient’s subscriber ID card at least annually and/or as each patient’s plan renews, and/or as often as necessary to assure the provider has the most current insurance information. 


Copies of the insurance cards must be kept on file in the patient’s healthcare record. 



At each visit, the provider should ask if there have been any changes to the patient’s insurance information.

HNS will gladly assist in determining if claims for a specific plan should be sent to HNS. Please use your HNS Fax Inquiry Form and fax a clear, legible copy of the member’s ID card to your HNS Service Representative. Your HNS Service Representative will notify you if the member’s plan should be filed through HNS.
